A great way to help speed through the intake process is to use our Download Intake Forms feature where you can complete the needed forms before you even get to the office. Just click on a form icon below and that form will open in a new tab. You can then view it online, download it, print it and fill it out in the comfort of your home without feeling rushed.
No printer? No problem! If you're not able to print these forms ahead of time, they will be provided at the time of check-in.
All patients should complete form 1) Review of Symptoms and either form 2) New Patient Questionaire OR form 3) Established Patient Questionaire, depending on your current patient status.
